Services

New Opportunities Wavier

Provides services to people with developmental disabilities, ages 3 and older, who choose to remain living in the community. Must be on the DD Request for services Registry. Contact your local Regional OCDD Office, District or Authority for information.

Childern's Choice Waiver

Provides supplemental supports to children with developmental disibilities, birth through 18 years of age, who currently live at home or who will leave an institution and return home. Individual must be on the DD request for Sevices Registry. Contact your local Regional OCDD Office, District or Authority for information.

Supports Waiver

Provides employment services for individuals with developmental disabilities, age 18 or older. These services consist of supported employment, day habilitaion, pre-vocational services, emergency respite, habilitation.  Individual must be on the DD request for Sevices Registry. Contact your local Regional OCDD Office, District or Authority for information. 

EPSDT Targeted Population Medicaid Program

Coordinates and assists in accessing services for Medicaid eligible (OCDD) participants from the ages of three up to twenty-one on the DD request for Services Registry or who are determined to meet special needs are eligible. Contact Statisatical Resources, Inc, for information about the program.

Community Choices Waiver

Provides services to elderly and disabled adults who meet admission requirements for a nursing facility and who can be served in the community.

Long Term- Personal Care Services

Provides assistance with activities of daily living for any qualified adult Medicaid recipient. Individual must meet the requirements for a nursing facility level of care. (In order for the participant to receive support coordination for their LT-PCS services it must be delivered in conjunction with the Elderly and Disabled Adult Waiver).

Adult Day Health Care Waiver

Provides day activities to include the following:  assistance with activities of daily living, health and nutrition counseling, health education classes, hot meal and two snacks, social services, transportation, some health and nursing services, and exercise programs.
